The Community Nurse may provide assistance in one or more of the following ways: 

• Coordinate care when several providers are involved
• Help elders continue to live safely and stay well at home
• Help follow the plan of care recommended by health care providers
• Link people with community resources and services to enhance daily life
• Reduce individual and family stress related to illness and aging
  
  

The Community Nurse does not provide hands on physical care. If skilled physical care or daily living assistance is needed, the community nurse will help the family explore options with home care agencies such as VNA, hospice care, or individuals in the community. The Community Nurse does not answer emergency calls. The Thetford Fast Squad  provides this service through the 911 dispatch.
